The Comprehensive Physician Portal is your answer. Comprehensive has been an innovator in the integration of technology into the practice of home health care. In 2004, we introduced telemonitoring to measure and provide you with your patients' vital signs while under our care. We are now pleased to introduce you to our Physician Portal - web site access to all of your Com prehensive Home Care patients under our care, in real time.

Improvement in the Coordination of Patient Care

Wonder how your patient is doing after surgery and during follow-up by a home health care agency?

By using Comprehensive's physician portal it is a simple matter of accessing your patient's account. Using Comprehensive's Physician Portal, you will be able to review, edit, electronically acknowledge, and sign plans of care and modifications thereto, and review current medications, nursing and therapy progress notes any time, at your convenience. This access enables you to intervene and make timely changes as necessary. Your patients' information is just a click away.

Medication Management

If adverse reactions to medications were classified as a distinct disease, it would be the fifth leading cause of death in the U.S. Among people over the age of 65, greater frailty increases their susceptibility to adverse medication effects.

Complicating matters is that most patients receive home health care following a hospital stay, during which their medication regime may have changed, resulting in greater compliance difficulties.

At the same time, the home health setting presents an opportunity to improve medication management. The reason is that these patients receive regular visits from skilled nurses who assess medication appropriateness, coordinate changes in drug regimes, and facilitate physician intervention as needed in a timely manner.

With the Comprehensive Physician Portal, you can readily access and assess a complete report on the medications your patient is taking.

Reduced Paper Flow and Increased Productivity

The process of coordinating physician authorizations and signatures for home health services is often a serious headache for home care providers and physicians alike. Most home health agencies rely on couriers, faxes or the mail to deliver plans of care, orders and other documentation to and from the referring physicians. As a result, physicians have mounds of paperwork piling up on their desks. This labor and paper intensive process may take from one week to one month just to obtain a simple signature and authorization.

To address this problem, Comprehensive has developed software for a Physician Portal - a secure HIPAA compliant, web-based solution - enabling physicians, via the internet, to securely access patient records, review clinical documentation, interact with home health clinicians and electronically sign treatment plans.

Comprehensive can now proactively prompt physicians when an authorization is needed, without tying up the physician's staff or fax. After clicking on an email link and logging in to the Comprehensive Physician Portal using the advanced ID authentication, physicians can create or modify orders and sign them with a simple mouse click. In addition, the Physician Portal produces documentation that helps the physician track time to safely bill CMS for Care Plan Oversight services.

Access to Patient Information

Accessing Comprehensive's Physician Portal is easy.

(2.) Click on Physician Access

(3.) Enter your unique and confidential Physician ID Access number provided to you by Comprehensive

Requirements

All you need is a computer that has internet connectivity (preferably with DSL, cable or high-speed capability) and IDs assigned by Comprehensive, which are issued upon signing an "Information Security Agreement." Information on the site can be accessed 24 hours a day, seven days a week from a physician's office or home computer. Using your mouse, you can navigate through an individual patient's clinical and de mographic data. Information displayed on the portal can also be printed to your local printer.

How do I get started?

Comprehensive will provide one-on-one training for physicians interested in using the portal. This orientation will include assignment of Physician Portal IDs, password creation, a demonstration of the portal features and location of data, and instruction on how to access the portal by physicians and office staff. Our support staff will be available to assist you with any questions or assistance you require to access the data, and troubleshoot, if necessary.

Privacy and Security

Comprehensive is committed to an effective compliance program, including laws and regulations governing the security of patient data. The Health Insurance Portability and Accountability Act (HIPAA) contains several provisions that protect the privacy and security of health information. The use of the Physician Portal will not impact how we handle your patients' information. Our privacy practices are outlined in a document "Notice of Privacy Practices" which is provided to all of your patients.
