Flu Shot Program

• Coordinate an annual flu shot program open to all residents.

Communications Program

• Conference meetings and feedback with Senior Living Community staff on the services being provided and trends in the Community
• Senior Living satisfaction surveys performed on a quarterly basis

Resident Assessment and Follow-Up

• Follow up with residents before, during and after hospitalization, nursing home stay, or physician visit to ensure the appropriate level of care
• Assessment for residents at their request for home care services
• Emergency response assistance available at the Senior Living Community's request
• Care review with resident's physician as needed and if requested
• Coordination of services as provided by other health care professionals, if requested
• Secure resident information is paramount to all services and will be held in the strictest of confidentiality and HIPAA compliance
